The Sherard Road Medical Centre

HOW DO I...
MAKE AN APPOINTMENT?

Routine Appointments

We offer appointments with the doctors and nurses on mornings, afternoons and evenings between 9.00am and 6.20pm.
See Surgery Times for more details.

Urgent Appointments

At the start of each session we allocate a number of emergency appointments which are usually at the end of surgery, although a few may be slotted in between booked patients. These appointments can only be booked on the day. Afternoon emergency slots are not available until after 12.30pm.

If you do need an emergency appointment, you may not have a choice of doctor or time. If, however, you wish to specify a particular doctor or timescale, we will try and fit you in with the earliest non-emergency appointment.

Alternatively, there is a minor injuries unit at Queen Elizabeth Hospital and an emergency eye clinic at Queen Mary's Hospital.

HOW DO I...
REQUEST A
HOME VISIT?

If you are too ill to attend the surgery you can request that a doctor visits you. All calls should be made before 11.00am, unless it is an emergency. On occasion you may be asked to hold to speak to a doctor.

Visits are usually undertaken between the end of morning surgery and the start of afternoon surgery. We are unable to promise that a particular doctor attends you. We will, however, try our best to ensure that you have continuity of treatment.

HOW DO I...
TELEPHONE FOR ADVICE?

If you have any queries about test results, hospital appointments, ambulance bookings etc, please ring after 2.00pm. This will ensure that you are more likely to be able to get through to speak to us.

HOW DO I...
REGISTER WITH THE PRACTICE?

We accept all patients living within the practice boundaries. We only register children if they are living with their legal carer. To register please bring your medical card to the surgery, detailing any changes as appropriate. If you do not have a medical card we will ask you to complete a registration form. Please also bring photo ID (if available) and utility bill (not mobile phone bill) with your current address. You will be asked to complete a medical questionnaire and we will make you an appointment to see a nurse for a new patient medical. A few weeks after your registration you should receive a new medical card.

All patients are registered with a named doctor and this doctor's name will appear on your medical card. You are, however, free to choose which doctor you wish to see. In an emergency we cannot guarantee that you will be able to see the doctor of your choice.

HOW DO I...
CHANGE MY PERSONAL DETAILS?

If you change address or telephone number please let us know as soon as possible. We need both your landline and your mobile numbers.
If your new address is outside our practice area, you may have to register with another, more local doctor.
If you change your name we will need documentary proof, eg marriage certificate or deed poll form.
