HOW DO I...
OBTAIN A REPEAT PRESCRIPTION?

We run a computerised repeat prescription system. Requests should be made in writing and posted in the boxes at reception, outside the door, or in the secure box at the gate. Please use the re-order form at the side of your prescription or write a note clearly spelling all the drugs you require. You can also post your request into us (if you want it sent back to you, please enclose a stamped addressed envelope). Alternatively, the following local chemists operate a collection service from the practice.

Prescriptions are normally ready 48 working hours after receipt of the request. Please ensure that you submit your request a few days before you are likely to run out of drugs.

We do not take requests for repeat prescriptions over the telephone.

OBTAIN A
SICKNESS CERTIFICATE?

The government provides a self-certificate form which covers you for the first seven calendar days of illness (these forms are available from reception). Thereafter you will need to see the doctor to get a medical certificate. Please note requests for appointments for medical certificates are not treated as an emergency. If you think your illness will continue after the self-certification period, please make an appointment for a date as soon as possible after the self-certification expires. The doctors do not issue medical certificates without seeing patients. If your employer does not accept the self-certificate form you will need to make an appointment for a private medical certificate. There
